Transfer Away From Lewis - OPT/ STEM OPT Students

Required documents:

1. OPT EAD card
2. Copy/screenshot of your Nelnet account showing $0.
1. Contact the Student Accounts Office (studentaccounts@lewisu.edu) if you have questions about your Nelnet account/ statements.
3. Copy of your new school’s admission letter
4. Completed º£½ÇÉçÇø Course Withdrawal and SEVIS Transfer Out Form

Process:

1. Upload all 4 required documents on the ISGSS: F-1 Transfer Out Google Form linked below.


2. You will receive a confirmation message after submitting the form. ISGSS will contact you if further documents are required. 
3. You will receive a confirmation email from ISGSS once your documents are approved. Processing time for all student requests is 5 business days, not including weekends and holidays.

Required documents:

1. Completed º£½ÇÉçÇø Course Withdrawal and SEVIS Transfer Out Form
2. STEM OPT EAD Card
3. Current employer's I-983 (all pages) with final evaluation on page 5 completed
1. If you are unsure about the evaluation dates on page 5 of your i983, email ISGSS@lewisu.edu and include your STEM EAD and official STEM employment start date with your previous employer before you submit the google form for a faster processing.
4. Copy or screenshot of your Nelnet student account showing $0

5. New school's admission letter

Process:

1. Upload all 5 required documents on the End of STEM Employer + Transfer Out Google Form linked below.


2. You will receive a confirmation message after submitting the form. ISGSS will contact you if further documents are required. 
3. You will receive a confirmation email from ISGSS once your documents are approved. Processing time for all student requests is 5 business days, not including weekends and holidays.
